Do you provide supplements?

  • GLIM provide supplements through Fullscript, and online dispensary, and extends a 25% discount to all clients and their families. Learn more on our supplements page

Can you be my primary care provider?

  • No, GLIM serves as an integrative and functional medicine consulting-only practice, and welcomes collaboration with other members of your healthcare team. You need to maintain your relationship with your primary care physician. GLIM focuses on getting to the root cause of your illness and guiding you on a personalized healing path.

Appointments 

How do I make an appointment?

  • First, please schedule a free phone consultation to learn more about GLIM. After deciding we are a good fit, GLIM will send a link to create your patient portal account and get you scheduled.

What should I expect at my initial appointment? 

  • Expect an appointment unlike one you've ever had before. After you have a free consult (see below), GLIM will schedule your first appointment. You will need to complete all forms online two weeks prior and submit payment one week prior to your scheduled appointment. Our goal is to learn as much as possible about you in advance of our meeting so that your appointment can be focused on developing your personalized healing plan. Your appointment will last 120 minutes and allow for in-depth conversation, analysis, and leave you with a few health action steps while you're waiting for your final healing plan from GLIM. 

Payment

What is the cost? 

  • GLIM's Integrative Medicine Consultation is $600 and includes the following: 

    • 120 minute initial consult

      • Extensive history​

      • Functional medicine lab-work plan

      • Actionable steps for client

    • 60 minute follow-up appointment

      • Review lab-work 

      • Create ongoing healing plan for client

  • Payment plans are available

    • A sliding payment scale is available as GLIM believes that integrative medicine should be available for all. 

    • Discounted rates are available for first-responders, healthcare workers, military, teachers, and seniors.

Is laboratory testing included in the cost?

  • GLIM offers both traditional and functional lab testing that is submitted by the laboratory directly to your insurance for reimbursement. Unlike other practitioners, GLIM does not make money on any lab work.

Do you accept insurance?

  • ​GLIM chooses to offer a personalized medical approach with extended medical visits. This is not rewarded under current insurance models. While GLIM does not accept insurance, we do provide a "superbill" with appropriate billing codes that you can submit to your commercial insurance for possible out-of-network benefits. You can check with your employer to see if HSA funds or flex spending accounts can be put toward GLIM services.

  • GLIM is not a participating Medicare or Medicaid provider. GLIM has "opted-out" which means that in order to see GLIM you must sign a contract that states you will not seek reimbursement of any of our services.

What kind of payment do you accept?

  • GLIM accepts cash, check, or Zelle as payment.

Where are you located?

  • You can meet with GLIM in the comfort of your own home through house visits or telemedicine appointments or visit our convenient locations in Chicago (IL), Evanston (IL), LaPorte (IN) and Valparaiso (IN).

Do you have a cancellation policy?

  • GLIM requires 24 hours notice of cancellation to refund your deposit. Those who do not provide 24 hours of notice will be charged 100% of the scheduled consultation fee due to the time and effort already put in to prepare for your appointment.

Do you offer telemedicine?

  • Yes! If you are a new patient, please schedule a free phone consultation to learn more about GLIM. After deciding we are a good fit, GLIM will send a link to create your patient portal account and get you scheduled.
